Check-out Procedure

Employee clearance is an official checkout procedure required by the University for all departing faculty and staff. Final clearance from Telecommunications & Network Services (TNS) cannot be granted until the following information has been received.

Clearance Steps for Academic Personnel:

  1. Email Clearances for Faculty via facout@csusb.edu
    • Assigned departmental requester should include the faculty member’s name, extension number, whether he/she is *returning and any special instructions, i.e., leave as is until (date), do not delete voice mail/email until (date), etc.
    • If voice mail is setup for departing faculty member, is this service to be deleted or left active.
  2. Clearance Forms for Faculty
    • Please include faculty member’s name, extension number, whether he/she is *returning and any special instructions, i.e., leave as is until (date), do not delete voice mail/email until (date), etc.
    • For departing faculty members with voice mail, is voice service to be deleted or left active.

*Please note: If this information is not provided, TNS will assume the faculty member is not returning and will take appropriate action.

Steps for Exiting Telecommunications for Staff/MPP:

  1. Clearance Forms for Staff/MPP
    • Please include staff member’s name, extension number and any special instructions, i.e., leave as is until (date), do not delete voice mail/email until (date), etc.
    • For departing staff members with voice mail, is voice service to be deleted or left active.

Other Helpful Information for TNS:

If the vacated extension has been reassigned, please provide the new person’s name, email address, title and whether voice mail is to be setup.
