Change of Major For New Students

Utilize the provided Change of Major instructions for a seamless process in submitting your request. If you're seeking to switch to an impacted program, ensure transcripts are on file for impaction eligibility verification. Date of Birth Change

This form is for all applicants who would like to update/change their Date of Birth. This is most common for students who incorrectly reported their Date of Birth while applying to CSUSB. A correct Date of Birth is required for the activation of your myCoyote account.

*Documentation is required: State or Federally issued Identification or Birth Certificate may be submitted. Use the link to upload documents directly through your myCoyote log-in:

Early Winter Starter Application (EWS) Closed

The Early Winter Starter (EWS) Application is designed for incoming students to the Spring term who wish to get a head start on their courses and start at CSUSB in the Winter. The EWS Application is only available to students who have accepted their Spring offer of Admission and are not otherwise enrolled in courses for the winter term at any other institution. EWS applications will not be fully processed until Final Official transcripts are submitted and Spring admission has been finalized.

*Please note: Submitting an Early Winter Starter Application does not affect your Spring Admission decision in any way. Your EWS application will not be fully processed until your final transcripts have been received and your Spring admission has been cleared by the Office of Admission and Student Recruitment.

Early Summer Starter Application (ESS) Closed

The Early Summer Starter (ESS) Application is designed for incoming students to the Fall term who wish to get a head start on their courses and start at CSUSB in the Summer. The ESS Application is only available to students who have accepted their Fall offer of Admission and are not otherwise enrolled in courses for the summer term at any other institution. ESS applications will not be fully processed until Final Official transcripts are submitted and Fall admission has been finalized.

*Please note: Submitting an Early Summer Starter Application does not affect your Fall Admission decision in any way. Your ESS application will not be fully processed until your final transcripts have been received and your Fall admission has been cleared by the Office of Admission and Student Recruitment.

Faculty/Staff/Dependents Fee Waiver

Members of the faculty and staff at Cal State San Bernardino and their dependents may be eligible to participate in a system-wide fee waiver program that allows for career and personal development offered through the CSUSB Human Resources Department. For details on your eligibility and the required forms, visit the Human Resources Department fee waiver page.

If you plan to enroll in courses at CSUSB for your own personal growth and you are not seeking any degree, complete and submit the Non-Degree Seeking/Transitory Student Application to the Office of Admissions and Student Recruitment in University Hall, Room 107 or fax (909) 537-7034.

Eligible individuals who wish to enroll in courses at CSUSB into a degree program must complete and submit a CSU application at Cal State Apply. Students must be eligible for admission to the university.

Legal Name Change

 This form is for incoming new CSUSB students who would like to change their "First, Middle, and Last Name" prior to course registration.

*Documentation is required: Birth Certificate, State or Federally issued Identification or Marriage License may be submitted.

Please Note: All continuing students and incoming students who have already registered for courses you will need to submit a Name Change Request with the Office of Registrars.

Non-Degree Seeking Student Application - Transitory Student

 The transitory application for CSUSB is available to students who are part of special programs allowing them to attend courses without being fully admitted to CSUSB. Transitory programs are for students who do not intend to graduate with a degree from CSUSB. There are several types of non-degree seeking/transitory programs at CSUSB so please be sure to check the box for your desired program.

Student Data Changes

Students are able to manage many aspects of their Personal Information through their myCoyote account. In order to do this, please log onto your myCoyote account and navigate to the to Personal Information tile. Once there you can manage the following personal information items:

  • Home & Mailing Address
  • Preferred Name
  • Phone number
  • Personal email address
  • Emergency Contacts

Items that cannot be updated and that require submitting a form along with documentation include:

  • Legal Name
  • Social Security
  • Date of Birth
  • Major Change 
Western Undergraduate Exchange (WUE)

 The WICHE/WUE program allows out of state applicants from the participating western region to attend CSUSB and pay no more than 150% of resident tuition fees while retaining their home state residency status. Students must meet all admission requirements and be otherwise eligible for admission to CSUSB as well as submit the WICHE/WUE application directly to CSUSB.
