Van Online Checkout Request

Please fill out this form to request a van for your department. Make sure all information is accurate. You will receive a confirmation email once your request is approved or denied.

The van must be returned to Lot A with a full gas tank. Failure to do so may result in additional fees and penalties. or restriction from using fleet vehicles. Ensure all forms are up-to-date as a state volunteer. If your forms are outdated, your request will be denied. Additionally, you must have completed the Recreation and Wellness driving training and the online vehicle daily check form before leaving campus.
