Dependent Eligibility Verification (DEV) is the process of verifying the eligibility of your spouse, domestic partner, children, stepchildren, and domestic partner children enrolled for state health and dental benefits enrollment. California Government Code section 22843.1 requires your employer to verify the eligibility of your dependent(s) at least once every three years to ensure only eligible dependents of State employees are enrolled in employer-sponsored coverage.
Required Documents
- A copy of the government issued marriage certificate or domestic partnership registration filed with the California Secretary of State or a comparable agency in another jurisdiction listing the family member as the domestic partner.
AND EITHER
- A copy of the first page of the subscriber's federal or state income tax return from the previous tax year listing the subscriber and the spouse or domestic partner.
OR
- A combination of other documentation, including but not limited to, a household bill, account statement, or insurance policy listing the name and address of the subscriber and the spouse or domestic partner, or other documents that substantiate the existence of a current marriage or domestic partnership. Household bills and account statements older than 60 calendar days are unacceptable.
The first document establishes the life event allowing the enrollment of the dependent (i.e., marriage or registering as domestic partners). The second document substantiates the relationship is current.

- Upload all required documents (Marriage License, Birth Certificate, Tax Return, household bills, etc.)
Verifying Family Members
Ninety (90) calendar days before your birth month, CalPERS will send you a letter with the verification due date, a list of the enrolled family members you must verify, and the acceptable verification documents. You are to promptly provide verification documents to your HR Benefits Department. CalPERS will verify the same family members for dental benefits.
Processing can take up to 30 days and you will receive a notification letter. If you don't respond or provide the required documents to your HR Benefits Department during your verification cycle, CalPERS will remove your family members from your health and/or dental benefits.
CalPERS informs you the month before your birth month in writing if we administratively remove your family members from health and dental benefits. We will include Consolidated Omnibus Budget Reconciliation Act (COBRA) continuation of coverage information with this letter. Your family members' disenrollment takes effect the first of the month after your birth month. If you provide verification documents for disenrolled, eligible dependents after the verification due date, CalPERS will re-enroll the family members prospectively for health and/or dental benefits. This may result in a gap of benefit coverage.
Verification Schedule
Your family members' eligibility will be verified once every three years. The verification cycle is based on your birth month. The following chart illustrates the schedule

This three-year cycle repeats. If you enroll family members within six months of your birth month, their eligibility will be verified during your next cycle.
